The following questionnaire includes a series of questions to be answered about your child. We ask you to respond honestly. All information provided will be confidential and used exclusively for the SPIS admission process.

Please answer all questions as best as you can, even those that may not fully apply to your case. If you have any questions, please consult the person in charge on the day of the evaluation.

1. Does the child respond when an adult initiates social contact, attempt to start a conversation, or willingly join an activity or conversation?
2. Does the child choose their friends? For example, when interacting with other children, does the child initiate and maintain friendships with certain children?
3. Does the child participate in competitive games that involve skill or endurance, winning, or achieving something?
4. Does the child act as a leader in relationships with others? Examples: Advises others on how to do something, leads specific activities, helps organize and set rules in games, or is sought out by peers for help or suggestions.

6. Does the child seek an adult’s help to resolve conflicts with peers?
7. Does the child take responsibility for their mistakes?
8. Can the child remove any clothing item (sock, glove, shoe, etc.)?

11. Can the child get water without asking for help? (Knows how to hold a cup, open and close the faucet, and fill the cup or glass without spilling too much.)
12. Does the child usually sleep through the night without wetting the bed? (They may call for help or go to the bathroom on their own.)
13. Does the child dress and undress independently (at least 5 times per week) without assistance?
14. Can the child button their clothes independently? How many buttons can they fasten?
15. Is the child able to stay at school or other familiar places alone or with friends without the company of an adult?
16. Is the child responsible for small amounts of money?
17. Is the child responsible for small household chores?
18. Does the child recognize themselves as the cause or reason for an action? (For example: turning on the light or TV, stacking objects and then knocking them down to observe the result.)
